Is it gynecomastia or chest fat?
Two conditions look alike. Pseudogynecomastia is fat only and responds to weight loss and liposuction. True gynecomastia is breast gland tissue that sits under the nipple as a firm disc. Most men who come for surgery have a mix of both.
Common causes are hormonal changes at puberty that don't fully settle, weight gain, anabolic steroids or bodybuilding supplements, some medicines, and rarely liver, kidney or hormone problems. Dr. Vikrant checks for these at the consultation and orders tests when the history suggests one.
Grades and the right operation
| Grade | What it looks like | Usual operation |
|---|---|---|
| 1 | Small lump behind the nipple | Gland removal, with or without liposuction |
| 2 | Moderate enlargement, no loose skin | Liposuction plus gland removal |
| 3 | Moderate enlargement with some loose skin | Liposuction plus gland removal; skin often tightens over 6 months |
| 4 | Large breast with drooping skin | Gland removal with skin excision |
The operation
Under general anaesthesia, Dr. Vikrant first removes the fat with liposuction to smooth the edges of the chest. The firm gland is then taken out through a small cut at the lower border of the areola, leaving a thin layer so the nipple doesn't sink. Cuts are closed with fine stitches and you wake up in a compression vest.
Recovery
The full timeline, with when to bathe, drive and restart chest workouts, is in gynecomastia surgery recovery week by week.
- Days 1–3: chest feels tight and bruised. Walk normally; avoid lifting arms above the shoulder.
- Day 3–5: back to office work and college.
- Weeks 1–6: wear the compression vest day and night.
- Week 4: light gym. Chest exercises from week 6.
- Month 3–6: swelling settles and the final flat contour shows.
Planning around a wedding or recruitment medical
Swelling takes about 3 months to settle enough for photos, and healing for a physical examination takes 4–6 weeks. Book surgery at least 3 months before a wedding and 2–3 months before a recruitment medical. Belly or flank fat can be treated with liposuction in the same sitting.

Common questions
What is the cost of gynecomastia surgery in Ranchi?
Gynecomastia surgery with Dr. Vikrant starts at ₹50,000 for both sides. Larger chests that need skin removal, or surgery combined with belly liposuction, cost more. You get the exact figure after examination.
Who is the best surgeon for gynecomastia in Ranchi?
Gynecomastia surgery should be done by a plastic surgeon who removes the gland as well as the fat. Liposuction alone often leaves a firm lump behind the nipple. Dr. Vikrant Ranjan, MCh plastic surgeon, combines both in one operation in Ranchi.
Will gynecomastia go away with gym or diet?
Fat on the chest reduces with weight loss, but true gland tissue does not. If you have a firm, rubbery disc under the nipple that stays after losing weight, exercise will not remove it. Teenage gynecomastia often settles by 18–19, so surgery is usually advised after that.
Will there be a visible scar?
The liposuction cuts are 3–4 mm, usually in the armpit or the side of the chest. The gland is removed through a semicircular cut on the lower edge of the dark areola, where the scar blends in. Very large chests may need longer scars, which is explained before surgery.
Can gynecomastia come back after surgery?
Removed gland tissue does not grow back. Steroid or anabolic supplement use, some medicines and large weight gain can enlarge the remaining tissue, so avoid these after surgery.
Is gynecomastia surgery safe for the Indian Army, police or job medicals?
Many young men in Jharkhand have this surgery before recruitment medicals. Healing takes 4–6 weeks. Plan the surgery at least 2–3 months before a medical examination, and check the specific rules of the recruiting body.
Do I need blood tests or a scan before gynecomastia surgery?
Everyone has routine pre-operative blood tests. Hormone tests or an ultrasound are added when the history or examination suggests a cause beyond puberty or weight gain, such as recent onset in an adult, one side growing quickly, a hard lump, nipple discharge, or certain medicines. A hard, fixed lump on one side is examined before any cosmetic surgery is planned.
Can surgery be done on one side only?
Yes. Gynecomastia is sometimes much larger on one side. Surgery can treat one side, or treat both sides by different amounts so they match. A lump on one side is always examined first to rule out other causes.
