Conditions treated

Acute injuries

  • Cut tendons (flexor and extensor) from glass, knives and machines
  • Cut nerves causing numbness of fingers
  • Amputated fingers and thumbs (replantation)
  • Crush injuries from machines, doors and road accidents
  • Skin loss over fingers and the hand needing flap cover
  • Fractures of hand bones combined with soft-tissue injury

Later problems

  • Stiff or bent fingers after old injuries
  • Tendon transfers for nerve palsies (radial, ulnar, median)
  • Burn contractures of the hand and web spaces
  • Nerve compression (carpal tunnel). See carpal tunnel syndrome: when surgery helps
  • Lumps such as ganglions and nerve tumours. Dr. Vikrant has published a case of a rare schwannoma of the plantar nerve, a nerve tumour of the same family.

Replantation: the golden hours

A cleanly cut finger can often be reattached using microsurgery to join arteries, veins, nerves and tendons under a microscope. Success depends on time and on how the part was carried. Fingers kept cool can survive up to 12 hours, sometimes longer; larger parts like a hand survive less time because they contain muscle. Read finger replantation: the golden hours.

Hand therapy

Surgery repairs the structures; therapy restores the movement. After tendon repair you will wear a splint and follow a set exercise programme for 6–12 weeks. Skipping therapy is the most common reason for a poor result.

Common questions

Who is the best hand surgeon in Ranchi?

In India, hand surgery is largely done by plastic surgeons and orthopaedic hand surgeons. For injuries involving tendons, nerves, blood vessels or skin loss, a plastic surgeon trained in microsurgery is the right choice. Dr. Vikrant Ranjan, MCh plastic surgeon, does hand and microsurgery at Jharkhand Charitable Hospital, Ranchi.

What should I do if a finger is cut off?

Wrap the finger in moist clean gauze, put it in a sealed plastic bag, and place that bag on ice in a second container. Never put the finger directly on ice or in water. Press on the hand wound and come to the hospital immediately. Call 090656 39577 on the way.

How soon should a cut tendon be repaired?

Ideally within a few days. Flexor tendons in the fingers retract and become hard to repair after 2–3 weeks, when a tendon graft may be needed instead.

Will I get full movement back?

It depends on the injury, the timing of repair and, very much, on hand therapy afterwards. Most clean tendon and nerve injuries repaired early recover good function. Crush injuries recover less completely.

Can old hand injuries be improved?

Often yes. Tendon grafts, tendon transfers, nerve repair or grafting, contracture release and joint fusion can restore function months or years after an injury.

What should I do for a firecracker injury to the hand?

Press on any bleeding with a clean cloth, don't wash the wound with force or pull out embedded pieces, cover the hand and keep it raised, and go straight to a hospital with a plastic or hand surgeon. Blast injuries often damage tendons, nerves and bone under the skin. See Diwali firecracker burns and hand injuries.

What should I do after a fodder cutter (chara machine) injury?

Press on the wound with a clean cloth, keep the hand raised, don't apply turmeric, lime or mud, and bring any cut-off part wrapped in damp cloth inside a sealed bag placed in ice water. Fodder cutter injuries crush and contaminate the hand, so they need cleaning in an operating theatre and early repair. Read the Hinglish guide ungli kat jaye to kya karein.

What are the signs of carpal tunnel syndrome, and when does it need surgery?

Tingling or numbness in the thumb, index and middle fingers, worse at night, is the typical sign. Mild cases settle with a night splint. Surgery, a short day-care release under local anaesthesia, is advised when numbness is constant, the thumb muscles weaken, or splinting hasn't helped. Read carpal tunnel syndrome: symptoms, tests and when surgery helps.

How soon can I use my hand after carpal tunnel surgery?

Light use such as eating, writing and using a phone is fine from the first day, and you move the fingers fully straight away. Stitches come out at 10–14 days. Most people return to desk work in 1–2 weeks and to heavy manual work in 4–6 weeks.
